On January 06, 2022, the Department of Health promulgated Official Dispatch No. 151/SYT-NVY on update on “Guidelines on home treatment packages for F0 cases” (version 1.7).
Pursuant to Decision No. 250/QD-BYT dated January 28, 2022, Decision No. 405/QD-BYT dated January 22, 2022, Decision No. 604/QD-BYT dated March 14, 2022 and Decision No. 616/QD-BYT dated March 15, 2022 of the Ministry of Health, the Department of Health updates “Guidelines on packages of drugs for home treatment for F0 cases” version 2.0 applicable to home treatment facilities for COVID-19 cases replacing Official Dispatch No. 151/SYT-NVY dated January 06, 2022. According to these guidelines, the Department of Health shall change home isolation conditions; shorten home isolation duration; change drugs for COVID-19 home treatment; adjust process for reception of F0 cases and confirmation of quarantine completion.
Heads of units assigned to provide care for F0 cases at home shall organize safe and effective implementation of this document.
Difficulties that arise during the period of implementation of this guidelines should be reported to the Ministry of Health (Health Care Operations – Tel: 028.3930.9981) for guidance./.

GUIDELINES ON HOME TREATMENT
PACKAGES FOR F0 CASES (VERSION 2.0)

- Home treatment facilities for COVID-19 cases: Health stations of communes, wards and commune-level towns; mobile health stations; health centers of districts, clinics; facilities engaged in treatment for COVID-19 cases.
- COVID-19 treatment facilities: COVID-19 Intensive care units (ICU), public and non-public hospitals, field hospitals for COVID-19 treatment and Level 1 treatment facilities (concentrated isolation area of F0 cases).
- Health workers and people engaged in home treatment for COVID-19 cases.
- COVID-19 cases (F0 cases) and caregivers of COVID-19 cases at home or F0 cases’ accommodation.
2. Home isolation conditions: F0 cases including children who fulfill clinical criteria and are able to take care of themselves or have caregivers.
a) Clinical criteria
(1) F0 case test positive for SARS-CoV-2 with RT-PCR or antigen rapid test using antigen rapid test kits licensed by the Ministry of Health performed by health workers/health facilities or by the F0 cases themselves or by F0 cases’ home caregivers.
- F0 cases do not have any symptoms or only have mild clinical symptoms such as fever, dry cough, sore throat, stuffy nose, fatigue, headache, muscle aches, numbness of the tongue, diarrhea, runny nose, loss of smell, loss of taste; normal breathing rate by age (children under 2 months old < 60 times/minute, children from 2 to 12 months old < 50 times/minute, children from 1 to 5 years old < 40 times/minute, children from 5 to 12 years old < 30 times/minute, children over 12 years old and adults < 20 times/minute), SpO2 ≥ 96% when breathing normally, no signs of shortness of breath and no respiratory failure.
- F0 cases do not have any underlying conditions or have underlying conditions which are being treated stably and wish to undergo home isolation.
(2) F0 cases who have been treated at COVID-19 treatment facilities for at least 05 days and have milder clinical symptoms, no fever (without taking antipyretic) before discharge from hospitals for at least 3 days and have positive test results with RT-PCR (CT < 30) or antigen rapid test may be discharged from these facilities and continue to stay in home isolation until the end of the 10-day period (test is not required to repeat).
b) Criteria of self-care and self-monitoring: F0 cases are able to take care of themselves (such as eating and drinking, taking bath, washing clothes, cleaning and monitoring their health according to guidelines of health workers, etc.); contact with health workers for monitoring and emergency if necessary. F0 cases who are children or people who need care must have their caregivers.
Notes: If a F0 case fulfills both criteria above but there are people in the high risk group (people with underlying conditions, people aged over 50, pregnant women, people who have not been fully vaccinated) living in his/her household, the F0 case is recommended to undergo isolation elsewhere (where there are no people in the high risk group or in a concentrated isolation facilities) to reduce the risk of spreading the disease to other members of the household, especially those in the high risk group.
3. Home treatment activities for F0 cases.
Activity 1. Identify and make lists of F0 cases undergoing home isolation
1. A commune-level health station shall receive F0 cases from various sources:
- For F0 cases who make declaration via https://khaibaof0.tphcm.gov.vn: the Commune-level health station shall assign a phone operator to receive calls about new F0 cases from 1022 hotline; log in the account “Ho Chi Minh City COVID-19 Management Digital Platform” for inspection and reception of the F0 within 24 hours. The commune-level health station must immediately contact, assess and receive F0 cases who are in the high risk group and have severe symptoms declared.
- For F0 cases or their caregivers who have directly declared to the health station by phone or in person: the health station must instruct these F0 cases to declare at https://khaibaof0.tphcm.gov.vn. In case F0 cases or their caregivers are unable to declare directly: the health station shall record and update F0 cases' information on the "Ho Chi Minh City COVID-19 Management Digital Platform” for management.
- For F0 cases who have not completely recovered from COVID-19 but are eligible for home isolation: the health station where F0 cases undergo isolation shall receive and monitor F0 cases' information on the "Ho Chi Minh City COVID-19 Management Digital Platform” transferred from COVID-19 treatment facilities.
2. The commune-level health station shall transfer list of people in the high risk group (including their phone numbers) to “Mạng lưới Thầy thuốc Đồng hành" ("Network of companion doctors”) for periodic healthcare advices and guidance on treatment for cases with underlying conditions (if any).
3. The health station shall propose the commune-level Steering Committee for COVID-19 prevention and control for promulgating a Decision on application for medical isolation measures for all newly home-isolated F0 cases received daily by the health station from the “COVID-19 Management Digital Platform”. F0 cases who do not completely recover from COVID-19 and are discharged from treatment facilities to continue to take care of their health at home shall undergo home isolation according to guidance of doctors specified in the F0 cases' discharge papers.
4. The health station shall transfer the list of home-isolated F0 cases to neighborhoods for monitoring the F0 cases’ home isolation.
Notes: On the basis of the number of home-isolated F0 cases in their commune, the above-mentioned steps may be performed by health workers of health stations, mobile health stations; community COVID-19 teams, mobile health teams, etc. The commune-level health station must develop cooperation procedures and provide treatment for F0 cases in the commune.
Activity 2. Provide guidance for F0 cases on self-care and self-monitoring
After receiving the list of F0 cases, the recipient shall quickly assess conditions for home isolation (clinical criteria and self-care), assess and classify risks according to Decision 5525/QD-BYT of the Ministry of Health in person or via remote media.
Provide guidance for F0 cases eligible for home isolation on important requirements, self-care and self-monitoring at home: (1) home essential items; (2) drugs for home treatment; (3) to-do list and not-to-do list; (4) monitor of their health and detection of severe symptoms for reporting to home treatment facilities for COVID-19 cases; (5) declaration of quarantine completion.
Activity 3. Monitor F0 cases’ health and provide F0 cases with examination and treatment
1. Monitoring F0 cases’ health
a) Daily monitor F0 cases’ health, update the developments on the “COVID-19 Management Digital Platform”.
b) Always prepare for reception of phone calls from F0 cases for giving advices or providing home examination and treatment in case these F0 cases need medical assistance or cannot be contacted.
2. Home examination prescription (appendices 3 and 4)
a) Drugs for home treatment for COVID-19 cases include 03 packages (A, B, C)
- Package A includes common drugs that are antipyretic drugs and health supplements.
- Package B includes anti-inflammatory drugs and antithrombotic drugs.
- Package C includes antiviral drugs used according to Ministry of Health's Program for controlled use of Molnupiravir in the community for COVID-19 cases with mild symptoms or purchased by COVID-19 cases as prescribed by their doctors.
b) Prescription for home treatment for COVID-19 cases
- For a F0 case in the high risk group or with mild symptoms, a home treatment facility for the F0 case must assess the F0 case’s health. If the F0 case needs to take antiviral drugs, his/her doctor shall prescribe as per regulations. The antiviral drugs must be used immediately after confirming diagnosis of COVID-19 and the best time to use of these drugs is within 05 days after symptoms onset.
- In case the F0 case feels shortness of breath (increased when exercising, resting breathing rate ≥ 20 times/minute or SpO2 < 96%), immediately contact the home treatment facility for assistance. The doctor assesses the F0 case's condition, if hospitalization is necessary; the doctor will give the F0 case single dose (package B) pending transfer to a treatment facility.
c) Dispensing of antiviral drugs
- For a F0 case undergoing home treatment who has a prescription of his/her doctor, the drugs in the prescription will be dispensed by the health station or purchased in pharmacies.
- For a F0 case subject to the risk group, his/her drugs will be dispensed immediately after receiving a positive test result. Prioritize the high risk group (people aged over 65 with underlying conditions). The F0 case must be instructed to take drugs when he/she is receiving these drugs before the health station decide to allow the F0 case to undergo treatment at home or at a treatment facility. In case the F0 case is transferred to a treatment facility, the health station shall instruct the F0 case to use the provided drugs; clearly specify instructions on drug use on the paper referral or notify the receiving facility for further monitoring.
- These drugs may be dispensed at home or at the health station and mobile health station after the F0 case’s family contacts them.
- When the F0 case use drugs dispensed from Ministry of Health's Program for controlled use of Molnupiravir in the community for COVID-19 cases with mild symptoms, he/she must sign a written consent form as prescribed.
Activity 4. Test F0 cases undergoing home isolation for SARS-CoV-2
1. Each F0 case, his/her caregiver or household shall undergo test for SARS-CoV-2 or test themselves at home when they have symptoms of COVID-19.
2. The F0 case can stop self-isolation after 07 days and recovering from COVID-19 (with a negative test result for SARS-CoV-2 with antigen rapid test using a antigen rapid kit test licensed by the Ministry of Health performed by a health worker/health facility or by the F0 case himself/herself or by the F0 case’s home caregiver).
3. If the test result on the 7th day is positive, the F0 case must stay in isolation until the end of the 10-day period applicable to cases who have been fully vaccinated or until the end of the 14-day period applicable to cases who have not been fully vaccinated as prescribed.
4. A home treatment facility for COVID-19 cases shall be responsible for confirming recovery from COVID-19 of the F0 case.
Activity 5. Emergency treatment and hospitalization of F0 cases undergoing home isolation
1. When a F0 case undergoing home isolation has severe symptoms, the home treatment facility for COVID-19 cases shall provide guidance, emergency treatment and contact to a rapid response team of the commune, district or Thu Duc city for emergency assistance and transfer of the F0 case to the nearest hospital.
2. Severe symptoms:
- In adults, difficult breathing includes shortness of breath, respiratory rate > 25 breaths/minute, hypersomnia, lethargy, pale lips or extremities, SpO2 < 94%.
- In children, tachypnea by age (< 2 months: ≥ 60 breaths/minute; 2 months - 12 months: ≥ 50 breaths/minute; 1-5 year(s) old: ≥ 40 breaths/minute, 5-12 years old: ≥ 30 breaths/minute, over 12 years old: ≥ 20 breaths/minute), nostril flare, pectus excavatum, lethargy, hypersomnia, breast/meal refusal, pale lips and extremities, SpO2 < 95% (if measuring device is available).
Activity 6. Complete procedures for home isolation for F0 cases
1. After a commune-level health station has received a message informing a F0 case’s test result on the 7th day from the 1022 hotline, an assigned health worker shall log into the account of the health station at https://covid 19.medinet.org.vn:75 and check.
- If the test result on the 7th day is negative, the health station shall confirm recovery from COVID-19 of the F0 case and complete procedures for confirming completion of home isolation.
- If the test result on the 7th day is positive, the health station shall provide guidance for the F0 case: (1) keeping self-isolation until the end of the 10-day period (for F0 cases who have been fully vaccinated) or until the end of the 14-day period (for F0 cases who have not been fully vaccinated); (2) doing test on the 10th or 14th day; (3) declaring test results of the 10th or 14th day.
2. In case any F0 case still not shows his/her test result on the 10th or 14th day, the health station shall contact the F0 case for confirming the test result or perform an antigen rapid test. If the health station cannot contact the F0 case, the case will be marked as "lost track" and closed.
3. Each treatment facility for F0 cases shall make a list of F0 cases who have recovered from COVID-19, report the list to the steering committee for COVID-19 prevention and control at commune level to implement procedures for issuing certificates of completion of home isolation updated on the "Ho Chi Minh City COVID-19 Management Digital Platform” as prescribed and submit the list to the health station at commune level to issue certificates of leave covered by social insurance to employees.
- Home isolation duration: at least 07 days from the day of receiving positive test results to the day receiving negative test results performed by health workers or F0 cases).
- Duration of leave covered by social insurance must be the same as the home isolation duration.
4. For F0 cases transferred to further home treatment by treatment facilities: Each social insurance agency shall make payment based on the number of leave days specified in the note of hospital discharge papers as prescribed in Clause 2, Article 21 of Circular No. 56/2017/TT-BYT dated December 29, 2017 of the Ministry of Health.

Guidelines on self-care and self-monitoring of F0 cases’ health at home
After declaring to the local health station, the F0 case has to satisfy requirements for home isolation, daily self-care and self-monitoring. To be specific:
1. Preparation for essential items for home isolation: Thermometer; fingertip pulse oximeter, sphygmomanometer (if any); medical face masks; means of hand hygiene; personal items; infectious waste containers with lids, means of communication (telephone).
2. Preparation for drugs for home treatment: Antipyretics for 3-5 days; electrolyte solution; cough suppressant (depending on symptoms) for 5-7 days with notes of indications, contraindications and cautions; physiological saline to gargle and rinse the nose for 5-7 days; drugs used for treatment for underlying conditions (hypertension, diabetes, cardiovascular disease, bronchial asthma, etc.) enough for 1-2 week(s).
- Create a separate isolation place. Keep it well ventilated, avoid using items that are difficult to clean (stuffed animals, paper, paperboard, etc.); do not use central air conditioner; keep windows open, usually clean surfaces, items and toilets.
- Do not be pessimistic, be comfortable. When facing difficulties, contact health workers for remote consultation. - Self-monitor health: Take body temperature, count breathing rates, measure SpO2 (if possible) at least twice a day or whenever feeling tired or shortness of breath.
- Keep using prescription drugs.
- Usually wear face mask, frequently wash or disinfect the hands.
- Rinse the nose and gargle daily with physiological saline.
- Frequently practice breathing and do exercise to improve health.
- Drink enough water or more if there is a fever or diarrhea. Do not skip meals, increase nutrients, eat foods containing enough nutrients, and eat more fresh fruits and vegetables.
- Disinfect items and surfaces such as hands, door handles, toilets and sinks, etc. Classify and collect infectious waste as prescribed.
- Have phone numbers of health workers of home treatment facilities for COVID-19 cases for contact if necessary; 1022 hotline, "Network of companion doctors” or rapid response teams at commune level or district level.
- Avoid leaving the isolation room, do not leave the house. Wear face mask and keep distance from others when leaving the isolation room.
- Do not share items with others.
- Do not share foods with others.
- Keep distance with other people or animals.
The F0 case’s caregiver and household/family have to always wear face masks, clean or disinfect their hands before and after taking care of the F0 case and keep distance with the F0 case.
- Difficult breathing, shortness of breath or increased respiratory rate (>21 breaths/minute for adults; ≥ 40 breaths/minute for children aged from 1 to 5; ≥ 30 breaths/minute for children aged from 05 to 12).
- Peripheral blood oxygen saturation (SpO2) < 96% (if measured).
- Rapid pulse > 120 beats/minute or < 50 beats/minute. Systolic blood pressure < 90 mmHg, diastolic blood pressure < 60 mmHg (if measuring equipment is available).
- Frequent chest pain, feeling of tightness in the chest and the pain increasing when taking a deep breath.
- Change of consciousness: confusion, lethargy, fatigue/exhaustion, excessive crying (in infants), hypersomnia and convulsion.
- Pale lips, extremities, toenails, skin, cold fingers and toes.
- Inability to drink or poor/reduced feeding, poor eating, vomiting (in children).
- Systemic dermatitis syndrome in children: high fever, red eyes, red lips, strawberry tongue, red swollen fingers and toes, etc. Acute diseases in children: Dengue fever; Hand, foot and mouth disease, etc.
- Other abnormal symptoms that are serious of COVID-19 cases.
6. Declaration of isolation completion
When the F0 case has stayed in isolation for 7 days (from the day of receiving positive test result), the following steps need to be performed:
(1) Take a photo of test result for SARS-CoV-2 performed on the 7th day by a health worker/health facility or by the F0 case himself/herself or by the F0 case’s home caregiver. If the test was performed by the F0 case or by his/her caregiver, write the F0 case’s name and test date on the test kit licensed by the Ministry of Health before taking photo.
(2) Access https://khaibaof0.tphcm.gov.vn (on computers or smart phones connected to the internet), choose "KB hoàn thành cách ly” (“Declare isolation completion”) to declare his/her test result on the 7th day, attached to the photo of the test result.
+ If the test result on the 7th day is negative, the home treatment facility for COVID-19 shall confirm recovery from COVID-19 of the F0 case and complete procedures for confirming completion of home isolation.
+ If the test result on the 7th day is positive, the F0 case shall stay in self-isolation until the end of the 10-day period (for F0 cases who have been fully vaccinated) or until the end of the 14-day period (for F0 cases who have not been fully vaccinated) and repeat the above-mentioned steps.
GUIDELINES ON USING COVID-19
DRUGS FOR HOME TREATMENT FOR CASES AGED OVER 18

COVID-19 drugs for home treatment include 03 packages (A, B, C); Package A includes common drugs that are antipyretic drugs and health supplements; Package B includes anti-inflammatory drugs and antithrombotic drugs only used in special cases; Package C includes antiviral drugs used according to approved clinical experimental doses.
1. Paracetamol 500mg
Take 01 tablet if the F0 case’s temperature is above 38°C, can be taken every 04-06 hours if the fever still persists.
2. Vitamins (multivitamin or vitamin C)
Multivitamin: 01 tablet/time/day.
OR
Vitamin C 500mg: 01 tablet/time and 02 times/day including 1 tablet in every morning and 1 tablet in every afternoon.
PACKAGE B (only used as prescribed by doctor according to Guidelines on diagnosis and treatment for COVID-19 cases prescribed in Decision No. 250/QD-BYT dated January 28, 2022 of the Ministry of Health.)
A F0 case who feels difficult breath (shortness of breath increasing when exercising, resting breathing rate ≥ 20 breaths/minute or SpO2 < 96%), immediately contact the doctor for assistance. The doctor who assesses the F0 case's condition. If hospitalization is necessary will prescribe anti-inflammatory drug corticosteroid and antithrombotic drugs (package B) for 1 day pending transfer to a treatment facility.
3. Dexamethasone 0,5mg x 12 tablets
12 tablets/time/day (equal to 06 mg)
OR
Methylprednisolone 16mg x 01 tablet
01 tablet/time/day.
4. Rivaroxaban 10mg x 01 tablet
01 tablet/time/day.
OR
Apixaban 2,5 mg x 01 tablet
01 tablet/time/day.
Note: Drugs No. 3 and No. 4 must not be used in the following cases: pregnant women and lactating women, people with one of the following diseases (peptic ulcer, liver failure, liver failure, kidney failure, bleeding disorders in the gastrointestinal tract, urinary tract and other bleeding disorders).
PACKAGE C (for 05 days) (for people aged 18 or above)
These are antiviral drugs for people with mild symptoms as prescribed by the doctor. The antiviral drugs must be used immediately after confirming diagnosis of COVID-19 and the best time to use of these drugs is within 05 days after symptoms onset. Prioritize the high risk group (including symptomatic and asymptomatic cases).
5. Molnupiravir 200mg or 400mg (tablets)
800mg in every morning and the same dose every afternoon, 02 times/day for 05 consecutive days.
OR
Favipiravir 200mg or 400mg (tablets)
1.600mg every morning and the same dose every afternoon on the first day.
600mg every morning and the same dose every afternoon on the second day to the 7th day.
Notes:
- When health facilities prescribe Molnupiravir for F0 cases, they shall instruct F0 cases to sign the written consent to participate in the Program for controlled use of Molnupiravir in the community for COVID-19 cases with mild symptoms before symptoms onset.
- Molnupiravir must NOT be used for pregnant women or women planning for pregnancy within 3 months or lactating women.
- People with liver failure, acute viral hepatitis, kidney failure, acute or chronic pancreatitis should ask doctors for advice before use.
- Favipiravir must NOT be used for pregnant women or women planning for pregnancy within 3 months or lactating women, people with severe liver failure, severe kidney failure; people with a history of gout should exercise caution when using this drug because it may increase uric acid levels and aggravate the disease.

GUIDELINES ON USING PACKAGES
OF COVID-19 DRUGS FOR HOME TREATMENT FOR CHILDREN

❖ Guidelines on home treatment for children with mild COVID-19
- Create separate isolation places for children, especially children with capacity for self-care, do not use central air conditioning; keep windows open.
- Follow standard precautions, wear masks for children aged ≥ 2, their caregivers, their family members/ households.
- Treat symptoms:
+ Take Antipyretic when the temperature is ≥ 38.5°C: Paracetamol with dose of 10-15mg/kg/time every 6 hours. Note: The total dose must not exceed 60 mg/kg/day.
+ Electrolyte solution: Oresol, rehydration packages or other electrolytes.
+ Cough medicines (prioritize herbal medicines), enough for 5-7 days.
+ Nasal drops: 0,9% natriclorua, enough for 5-7 days.
+ Anti-diarrheal medicines: probiotics, digestive enzymes.
+ Drugs for treatment for underlying conditions (if any, for 01-02 week (s)).
+ Cough medicines: prioritize herbal cough medicines.
Note: Do not use antiviral drugs, antibiotics, anti-inflammatory drugs, etc. without prescription. Steam therapy is not recommended for children.
- Drink a lot of water.
- Ensure adequate nutrition: breastfeeding, eating well.
- Keep personal hygiene (body, teeth, nose and throat).
- Do exercises at home and practice breathing for at least 15 minutes/day (for older children).
- Monitor:
+ Take children’s body temperature at least 2 times/day or whenever they have fever.
+ measure the children’s SpO2 (if any) at least 2 times/day or whenever they have fatigue, rapid breathing/shortness of breath.
- Daily monitor children’s health, report health workers if they have warning signs.
❖ Instructions to use packages of COVID-19 drugs for home treatment for children (for 07 days):

I hereby give my consent to participation in the “Program for controlled use of Molnupiravir in the community for COVID-19 cases with mild symptoms” and declare that:
• I will use the drug for the right purposes of the program and do not share them with others (including relatives). If the program's drugs are not used up, I will return them to local health workers (with confirmation of the deliverer - the receiver).
• I will check the name (Molnupiravir) before receiving and use it according to the instructions of health workers.
• I am not pregnant/ breastfeeding while using the drug or plan for pregnancy within 3 months from the last day of using the drug.
• I will immediately report to the commune-level health station or mobile health station if there are adverse effects during the use of this drug. I will not use wrong doses.
• I will take full responsibility if I use this drug for unintended purposes, against instructions or terms therein.
